A man in his late seventies with no past medical history presented to the hospital with sudden left sided weakness followed by decreased level of consciousness. Head CT showed no hemorrhage, no acute ischemic changes. Diffusion-weighted MRI showed infarcts in the right M2 territory.

Two hours before the onset, he had been participating in the annual festival of this town.

A cardiologist who provided initial care in the ER as a duty doctor called me. I was in the hospital. I went down there quickly.

He received Alteplase intravenous tPA.

He recovered a little transiently. He talked about his family, his scar in the left knee and his baseball life.
